• Complete the Bank Information Application Form
• State ID or Driver’s License
• Corporation or LLC documents
• EIN (Employee Identification Number) letter form the IRS
• EFIN Summary page
• PTIN Letter from the IRS

Please make sure you upload all the required documents listed above.  We will not be able to set-up your tax preparation software or the bank you will use to process your tax payer refund without the following documents listed above.  Please submit all the proper documents listed above.
